Therapeutic Ultrasound: The Lack Of Science
When I started studying for this article way back in the mid-2000s, I was quite surprised by how little there was to study. Back then, every scientific paper about US pointed out there is not enough research on this topic, or at least not enough good research and not much has changed. A 2015 review of ultrasound for rotator cuff tendinopathy found only six trials, all poor quality.
Thats not a lot to go on, and its typical. Its a bit shocking. Were talking about ultrasound, here: one of the staples of physical therapy! Its not a fringe treatment. It practically defines the experience of going to a physiotherapist. Everyone has had that cold gel slapped on an injury, and felt that tingling, penetrating placebo?

I appreciate all the support over the last 2 years while setting up my new practice. I have had successful results with over 90-95% of my patients with disc injuries/ neck/ back pain in utilizing the DTS table as a stand alone treatment. I will be honest- it was a tough decision initially because of the financial investment/ commitment while I was starting practice. I am glad that I decided to integrate the DTS into my practice from the start because of the referrals and the wonderful response from my patients. Thanks again for all your help over the last 2 years and continued support.

This is one of the biggest selling machines worldwide and one of the best ultrasound units for home use. In fact, we put it on the top of the list because it is worth the money. Additionally, this device offers professional results. With a frequency of 1 Mhz, it comes with a microprocessor-controlled digital group.
We were quite impressed by its continuous and pulsed application. As a result, it delivers a quick pain relief mechanism that matches the effect of a medical professionals touch. Furthermore, it is designed with a durable body and a comfortable wide transducer to alleviate pain from more significant parts. For this reason, we would like to specifically endorse this device for athletes to help reduce pain and improve performance.

A Study Of Ultrasound Therapy Shows A Clear Effect On Trigger Point Sensitivity
Quick muscle knot orientation: So-called muscle knots AKA trigger points are small unexplained sore spots in muscle tissue associated with stiffness and soreness. No one doubts that they are there, but they are unexplained and controversial. They can be surprisingly intense, cause pain in confusing patterns, and they grow like weeds around other painful problems and injuries, but most healthcare professionals know little about them, so misdiagnosis is epidemic. For more information about how trigger points might be involved in your own medical history, see PainScience.coms best-selling tutorial:
File this one under intriguing: Canadian researchers treated trigger points in 50 patients with either ultrasound or a sham, and the amount of pressure on the trigger point required to induce pain was measured before and after. Trigger points treated with ultrasound were more tolerant of pressure than those that were not treated, at 1, 3 and 5 minutes after treatment. The improvement was no longer significant just 10 and 15 minutes later, however so the effect in this case was brief. The authors concluded:
low-dose ultrasound evokes short-term segmental antinociceptive effects on trigger points which may have applications in the management of musculoskeletal pain.

What Is Tens Therapy Used For
TENS therapy can be used for a wide range of physical problems. A TENS unit for sciatica or a TENS unit for knee pain are 2 of the more popular ways to use one of these devices. Ultrasound therapy for plantar fasciitis is also possible with one of these units.
I personally have used TENS therapy for back and neck pain. You can also use a TENS Unit for pinched nerves to try and release the muscles that are pinching your nerve because a TENS unit relax muscles when the low voltage electricity is passes into them. Other uses include using a TENS machine for muscle knots, TMJ, anxiety, you name it. Transcutaneous electrical nerve stimulation uses really low voltage electricity shot into your muscles to relieve pain.
Is ultrasound good for gout? The jury is out on that one, because there havent been any conclusive studies. Consult your doctor if you think you have gout and ask them if they recommend an ultrasound unit.

How To Use A Tens Unit
Using a TENS Unit is so simple anyone can do it in the comfort of their home, office, in the car, or on vacation. I personally use mine in all those places! They are so small and discrete that no one will know youre using one.
You plug in the cords, depending how many you need. They typically come with 2 sticky pads on each cord. You plug in 1 cord and then you remove the plastic protector from the pads. Remember to keep them on the plastic when not in use to keep them sticky as long as possible. Use saran wrap if you lose the ones your unit came with.
Place the sticky pads on the area you need to heal. Use the TENS electrode placement chart pdf at this link to place your electrode pads in the right place for maximum results. Turn on your unit and select the type of setting that works best for you and the area youre treating. Most TENS units have several you can choose from like massage, acupuncture, cupping, deep tissue work, etc.

The Discouraging State Of What Little Ultrasound Evidence There Is
In most cases I consider ultrasound less than useless that’s 8-10 minutes wasted that could be used doing something that might actually help.
Jason Silvernail, DPT, Board-Certified in Orthopedic Physical Therapy, in an internet forum discussion
Ultrasound is an unusually easy treatment to test scientifically. If it works reasonably well, then the results should be pretty clear. Just compare results in patients who received real ultrasound to patients who get a fake instead! And yet there are just a few dozen such experiments in the scientific literature, and most of them are seriously flawed. Conclusions from evidence reviews like this one from van der Windt et al are typical:
As yet, there seems to be little evidence to support the use of ultrasound therapy in the treatment of musculoskeletal disorders. The large majority of 13 randomized placebo-controlled trials with adequate methods did not support the existence of clinically important or statistically significant differences in favour of ultrasound therapy.
van der Windt et al, 1999, Pain
Did not support the existence of? Ouch! Ultrasounds therapeutic effect has an existential crisis.

|Desmeules 2015||rotator cuff tendinopathy||negative review does not provide any benefit based on low to moderate level evidence from 11 weak trials|

Clinical Trials Of Ultrasound Cavitation For Body Sculpting
The clinical trials of ultrasonic cavitation for fat loss showed that treatment provides significant satisfying results in a circumferential reduction without any serious adverse effects . Patients had a reduction in fat tissue by 2 cm or more across the abdomen, flanks, hips, and thighs. The results were consistent with each device used and none are associated with any adverse effects. In the review from 2015, the data was gathered from 31 studies with a total of 2937 patients. 843 patients were treated withHIFU . All the studies varied in the treatment regimen, treated areas, and follow-up times, however, a vast majority of patients experienced positive results without any noticeable side effects .
In a study on 30 healthy patients , the efficacy and safety of HIFU were satisfactorily confirmed. The patients underwent 3 treatments, 1 per month , and were followed up 1-month post last treatment. The subjects were treated on the abdomen, thighs, flanks, inner knees, and chest . In the outcome, all patients showed a significant reduction in thickness of the fat layer within the treated area, and circumference reduction was 4.95+/-1.99 cm. However, the patients showed no change in their weight during the treatment and follow-up period. No adverse effects were observed either .
Additionally, only radiofrequency, low-level laser therapy and cryo lipolysis have been approved for use in the United States .

There are many types and forms of these machines from small hand-held wands to complex machines with many different probes and settings. However simple or complicated, the principle is the same. All machines have wands with metalheads transmitting high-frequency sound waves. To perform the treatment you need to press the wand to your skin lightly on the wanted area and move the wand imitating massage movements.
