All magnets are by nature bipolar, having a north and a south pole, usually located on opposite sides of the magnet. SMFs are sometimes likened to electrical fields and described as having a positive or negative charge corresponding to their south and north poles, respectively. This concept is inaccurate. Gauss meters read negative or positive depending on the orientation of the measuring probe toward the surface of the magnet, not on the actual polarity of a particular side of the magnet. As the gauss meter probe turns 180 degrees, the negative reading will become positive and vice versa.
Practitioners further caution that the bionorth pole should be applied only when treating painful or excess conditions and the biosouth pole applied for conditions of deficiency. Of clinical relevance is their strongly held contention that the biosouth pole should not be applied when treating areas with possible infection or tumor. To date, no RCTs have been reported in the peer-reviewed scientific literature that authenticate or negate these clinical testimonials. Nevertheless, based on the experience of respected acupuncturists, it seems prudent to refrain from applying the biosouth pole to areas of suspected infection or tumor.

Context: Low back pain is one of the most frequent and expensive medical conditions in the United States. It is estimated that 85% of people will complain of low back pain during their lifetime, and currently more than 5 million people are disabled with this condition.
Objective: To compare the effectiveness of one type of therapeutic magnet, a bipolar permanent magnet, with a matching placebo device for patients with chronic low back pain.
Subjects: Nineteen men and one woman with stable low back pain, a mean of 19 years duration, and no past use of magnet therapy for the condition.
Design and Setting: Randomized, double-blind, placebo-controlled, crossover pilot study conducted from February 1998 to May 1999 in a Veterans Affairs Hospital ambulatory care physical medicine and rehabilitation clinic.
Interventions: For each patient, real and sham bipolar permanent magnets were applied on alternate weeks for six hours per day, three days per week for one week with a one-week washout period between the two treatment weeks. The magnets were held into place with an abdominal binder, connected via velcro straps.
Application of one variety of permanent magnet had no effect on this small group of subjects with chronic low back pain.
Does Magnetic Therapy Have A Place In Pain Management
As with many other CAM therapies, a definitive answer is not available as to whether magnetic therapy is useful in pain management. There are anecdotal reports of efficacy for magnets aimed at diabetic neuropathy, burning feet syndrome, carpal tunnel syndrome, and headaches.
