What Is The Success Rate Of Proton Therapy

Safety And Efficacy Of Proton Therapy

Radiation Treatment and Proton Beam for Children with Brain Tumors – Mayo Clinic

Many people with locally advanced cancers are treated with a combination of chemotherapy and either traditional or proton radiation. For patients getting chemotherapy and radiation at the same time, finding ways to limit side effects without making the treatment less effective is a high priority, Dr. Baumann said.

He and his colleagues analyzed data from nearly 1,500 adults with 11 different types of cancer. All participants had received simultaneous chemotherapy plus radiation at the University of Pennsylvania Health System between 2011 and 2016 and had been followed to track side effects and cancer outcomes, including survival. Almost 400 had received proton therapy and the rest received traditional radiation.

Those who received proton therapy experienced far fewer serious side effects than those who received traditional radiation, the researchers found. Within 90 days of starting treatment, 45 patients in the proton therapy group and 301 patients in the traditional radiation group experienced a severe side effectthat is, an effect severe enough to warrant hospitalization.

In addition, proton therapy didnt affect peoples abilities to perform routine activities like housework as much as traditional radiation. Over the course of treatment, performance status scores were half as likely to decline for patients treated with proton therapy as for those who received traditional radiation.

Positioning For Proton Beam Treatment

Positioning the body is very important. It can take several minutes for the team to adjust both you and the instruments to make sure the proton beam will reach the exact spot of the tumor.

While the treatment is happening, you must be very still on the table. The team will work with you to help keep you in place. For example, a custom-fitted mask can help those who are getting treatment for brain tumors. The mask is made of mesh that allows patients to see and breathe.

Proton Therapy In Reirradiation

It should be noted that a reduced integral radiation load in PT has certain prospects in reirradiation . In 2016, Romesser et al. represented a multi-center retrospective analysis, which involved 92 patients exposed to PT reirradiation using a passive scattered beam, from 2011 to 2014. During the first year, after therapy, loco-regional recurrence rate was 25.1%, while overall survival rate was 65.2%, and the recurrence rate of stage IIIV complications was 14.1%. Phan et al. in the same year represented the findings of proton reirradiation of 60 patients with HNC, and 45 of them were treated using IMPT: the recurrence-free rate and overall survival rate at the first year were 68.4 and 83.8%, respectively. Most notably, both research groups agreed that proton reirradiation is significantly less toxic compared to photon therapy.

Talk To Your Healthcare Provider

There is a lot of information to digest simply looking at the role of radiation therapy in the different types and stages of lung cancer, but individual differences are crucial as well. Every person is unique, and every lung cancer is different in some way.

If you are considering radiation therapy for lung cancer, talk to your healthcare provider. Some people like to chart out a list of the pros and cons of treatment. It’s as important to discuss potential side effects as the potential effectiveness of therapy. Your quality of life while going through cancer treatment is important.

There’s no correct answer when it comes to making a decision. Some people wish to do as much as possible, and others are more concerned about their quality of life. Some side effects may be very bothersome to one person but not another.

Making sure your doctor understands you and your goals as an individual is a great start in getting the care you deserve.

Cancers Treated With Proton Therapy

Proton therapy is useful for treating tumors that have not spread and are near important parts of the body. For instance, cancers near the brain and spinal cord. It is also used for treating children because it lessens the chance of harming healthy, growing tissue. Children may receive proton therapy for cancers of the brain and spinal cord. It is also used for cancer of the eye, such as retinoblastoma and orbital rhabdomyosarcoma.

Proton therapy also may be used to treat these cancers:

  • Central nervous system cancers, including chordoma, chondrosarcoma, and malignant meningioma

  • Eye cancer, including uveal melanoma or choroidal melanoma

  • Head and neck cancers, including nasal cavity and paranasal sinus cancer and some nasopharyngeal cancers

  • Lung cancer

  • Spinal and pelvic sarcomas, which are cancers that occur in the soft-tissue and bone

  • Noncancerous brain tumors

Advancing Proton Therapy Research For Head And Neck Cancer

Radiation oncologists, physicists, and biologists continue to collaborate to improve proton therapy. Based on current evidence and promising trends, we anticipate that proton beam therapy will become more effective and less toxic.

Multiple institutions are studying outcomes for patients with head and neck cancer treated with proton therapy. Table 1 in our paper summarizes the trial names, inclusions, treatments, and primary endpoints for 15 ongoing clinical trials. Three of the studies led by MSK are:

  • A clinical trial of endoscopic surgery followed by chemotherapy and proton radiation for the treatment of tumors in the sinus and nasal passages
  • A phase II study of proton re-irradiation for recurrent head and neck cancer

A phase II randomized study of proton beam versus photon beam radiotherapy in the treatment of unilateral head and neck cancer, in collaboration with the Mayo Clinic and Mount Sinai Hospital .

Cost Of Proton Therapy

Though proton therapy has been gaining ground all over the world, the major issue with availing of the procedure is the cost. No matter how effective the procedure is, not many people are able to avail the treatment due to the high investment that it involves. On average, proton therapy cost has been estimated to be ranging between $30,000 to $120,000. A number of people resort to alternative radiotherapy methods for two reasons: one involves the lack of exposure to the benefits of the process, and the other is difficulty in affording the procedure. However, the therapy may be covered in some medical insurances, which implies, that if the candidate is in possession of specific insurances which cover the expenses, availing the procedure becomes much easier.

How Proton Therapy Effectively Treats Prostate Cancer

How Proton Therapy Works

Proton therapy treats prostate cancer with high doses of radiation that are more accurate, and potentially more effective, than traditional radiation. Our targeted proton beams focus most of their destructive energy at the tumor site, therefore causing less damage to healthy surrounding tissue as they enter the body. Because of this, proton therapy patients dont have to worry about many of the side effects and additional healthy tissue damage that is commonly associated with X-ray therapy.

Proton beam therapy may cause fewer side effects than standard radiotherapy. This is because normal cells are exposed to less radiation. The possible side effects will depend on:

  • the area of your body being treated and what structures are close by
  • the amount of proton beam therapy you have
  • other treatments you are having, such as chemotherapy.

Below are some of the side effects of proton beam therapy. They will not affect everyone who has this treatment. Your cancer doctor, specialist nurse or radiographer will explain any possible side effects to you before you start treatment. It is important to tell them about any side effects you get.

Subacute And Late Toxicities

Despite proton therapy having a dosimetry advantage to photons for sparing normal structures outside of the treatment field, toxicities may still occur within or very close to the target. Multiple studies have shown progressively higher relative biological effectiveness at the distal edge of the proton Bragg peak, , which may land accidentally in critical organs due to the combination of suboptimal beam arrangement and range uncertainty.

As proton therapy is used for reirradiation, there are higher risks of normal tissue injury due to significant previous radiation contributing to potential subacute and late toxicities. Central nervous system toxicities can develop years later, be quite symptomatic, and significantly affect long term survivors quality of life. Temporal lobe necrosis is often seen after head and neck irradiation to the skull base, nasopharynx, and paranasal sinuses. Overall, there is an estimated five percent risk of developing late CNS toxicities after proton therapy for head and neck cancer. Notable clinical evidence of CNS toxicities is as follows:

  • McDonald et al. found that any grade of temporal lobe necrosis approached 12.7 percent at three years, with grade 2 or higher radiation necrosis occurring in 5.7 percent of patients.
  • A recent report by Kitpanit et al. found a two-year temporal lobe necrosis rate of 4.6 percent among 234 patients with a median time to necrosis of 20.9 months.

Non-CNS late toxicities after proton therapy have also been studied:

A standard procedure used to treat nasal and sinonasal tumors is surgery and further radiotherapy combined with or without chemotherapy depending on a disease stage. In this case, PT enables to achieve high total doses preserving patients good life quality. The Appendix represents the results of PT studies of HNC including this localization. The most clinical observations are included in the study by Resto et al. published in 2008. The investigation involved 102 patients who underwent treatment from 1991 to 2002. Most patients at the first stage underwent surgery followed by combined photon-proton therapy. The surgery radicality had an effect, primarily, on total survival rate , progression-free survival , and the risk of delayed-effect risk . In a group of patients with total tumor resection, local control index was 95%, in those with partial resection 82%, while in patients who had just undergone biopsy 87% . In general, treatment failures were primarily due to a delayed progression: 30% of patients within a five-year period were found to have some metastases.

Benefits Of Proton Therapy For Breast Cancer

There are several potential benefits of using proton therapy for breast cancer treatment. The proton beams used in proton therapy do not reach tissues beyond the cancerous tumor. This lowers the risk of normal tissues being damaged and cuts down on side effects. This is especially helpful for people with left-sided breast cancer because the tumor is so close to the heart.

Proton therapy also uses a higher dose of radiation, giving it a better chance of destroying the tumor. Possible benefits of using proton therapy for breast cancer include:

  • Fewer side effects than traditional therapy
  • Painless

Is Proton Therapy Treatment Optimal for Childhood Cancer?

One of the first symptoms of prostate issues is pain or tenderness in the groin or lower back. This can be the result of a noncancerous condition called enlarged prostatic tissue, or it could be an infection of the bladder. In either case, its important to see a doctor as soon as possible. If youre suffering from prostate pain, you may want to consider reducing your caffeine intake.

Another symptom of a potentially enlarged prostate is difficulty starting a stream of urine, leaking, or dribbling. These symptoms are not serious, but theyre still alarming. Most men put up with an enlarged prostate for years before seeking medical attention, but they typically seek treatment as soon as they notice symptoms. Even if you dont have symptoms, its worth getting checked to determine if you have any prostate issues.

If you experience nightly bathroom runs, you may be experiencing an enlarged prostate. You may be having difficulty starting a stream of urine, or you may even be dribbling or leaking during the day. These problems arent life-threatening, but can become a nuisance. You should not ignore these signs and seek treatment as soon as you notice them. If you feel any of these symptoms, you should consult a doctor.

Lung Cancer Survival Rates

It’s helpful to review the survival rates of the different types of lung cancer when looking at the potential benefits of radiation.

When reviewing these, however, it’s important to note that no two lung cancers are alike. Even if two people develop cancers of the same type and same stage, they may have considerably different outcomes depending on many factors, such as the gene mutations present in the tumor.

In addition, there are limitations to using statistics, including that people aren’t statistics. There is no “average” person who has an “average” journey with lung cancer.

Perhaps the most important limitation in discussing five-year survival rates with lung cancer is that treatments are improving and rapidly changing. The treatment you receive may not have existed five or 10 years ago, so the statistics may not be relevant to your case.

For example, for decades there was relatively little progress in lung cancer treatment. This began to change quickly, starting around the year 2010, first with targeted therapies and then immunotherapy. There was a new drug available every few months.

Even during the height of the COVID-19 pandemic, in May of 2020, six new medications or combinations of medications were approved to treat lung cancer. If you receive one of these treatments, statistics from 2010 to 2016 may mean little.

Non-small cell lung cancer is broken down into four stages, although three are important when discussing statistics, which are:

  • Local: 27%

Another type of prostate issue is chronic prostatitis, or chronic pelvic pain syndrome. This condition causes pain in the lower back and groin area, and may cause urinary retention. Symptoms include leaking and discomfort. In severe cases, a catheter may be required to relieve the symptoms. If the problem is unresponsive to other treatments, your doctor may suggest a surgical procedure. If these do not work, your symptoms could progress and become chronic.

An acute bacterial infection can cause a burning sensation. Inflammation of the prostate can affect the bladder and result in discomfort and other symptoms. This is the most common urinary tract problem in men under 50, and the third most common in men over 65. The symptoms of acute bacterial prostatitis are similar to those of CPPS. Patients may experience a fever or chills as a result of the infection.

A bacterial infection can also lead to prostate issues. Acute bacterial infections can be hard to treat. Some men with a bacterial infection may need to take antibiotics to prevent or treat symptoms. Symptoms of the disease include fever and chills, pain in the lower back and the tip of the penis. Some men may have blood in the urine, frequent urination, and blood in the urine. If you suffer from acute bacterial prostatitis, a medical professional should be able to prescribe you the appropriate treatments to prevent the disease.
