Radiation Treatment and Proton Beam for Children with Brain Tumors – Mayo Clinic

Many people with locally advanced cancers are treated with a combination of chemotherapy and either traditional or proton radiation. For patients getting chemotherapy and radiation at the same time, finding ways to limit side effects without making the treatment less effective is a high priority, Dr. Baumann said.

He and his colleagues analyzed data from nearly 1,500 adults with 11 different types of cancer. All participants had received simultaneous chemotherapy plus radiation at the University of Pennsylvania Health System between 2011 and 2016 and had been followed to track side effects and cancer outcomes, including survival. Almost 400 had received proton therapy and the rest received traditional radiation.

Those who received proton therapy experienced far fewer serious side effects than those who received traditional radiation, the researchers found. Within 90 days of starting treatment, 45 patients in the proton therapy group and 301 patients in the traditional radiation group experienced a severe side effectthat is, an effect severe enough to warrant hospitalization.

In addition, proton therapy didnt affect peoples abilities to perform routine activities like housework as much as traditional radiation. Over the course of treatment, performance status scores were half as likely to decline for patients treated with proton therapy as for those who received traditional radiation.

Most notably, both research groups agreed that proton reirradiation is significantly less toxic compared to photon therapy.

Temporal lobe necrosis is often seen after head and neck irradiation to the skull base, nasopharynx, and paranasal sinuses. Overall, there is an estimated five percent risk of developing late CNS toxicities after proton therapy for head and neck cancer. Notable clinical evidence of CNS toxicities is as follows:

  • McDonald et al. found that any grade of temporal lobe necrosis approached 12.7 percent at three years, with grade 2 or higher radiation necrosis occurring in 5.7 percent of patients.
  • A recent report by Kitpanit et al. found a two-year temporal lobe necrosis rate of 4.6 percent among 234 patients with a median time to necrosis of 20.9 months.

It’s helpful to review the survival rates of the different types of lung cancer when looking at the potential benefits of radiation.

When reviewing these, however, it’s important to note that no two lung cancers are alike. Even if two people develop cancers of the same type and same stage, they may have considerably different outcomes depending on many factors, such as the gene mutations present in the tumor.

In addition, there are limitations to using statistics, including that people aren’t statistics. There is no “average” person who has an “average” journey with lung cancer.

Perhaps the most important limitation in discussing five-year survival rates with lung cancer is that treatments are improving and rapidly changing. The treatment you receive may not have existed five or 10 years ago, so the statistics may not be relevant to your case.
